Parents and God parents who have been scheduled for a baptism are asked to watch this brief video before the day of a scheduled baptism. Once viewed, all participants are asked to fill out the form below individually.
If you are interested in having your child baptized at St. John’s please schedule an appointment with the pastor. You are welcome to fill out the Baptism request form below, however please be aware that the baptism form isn’t approved until the pastor reviews the form and confirms it on his schedule.  Â
